Logins the hospital controls
Nobody signs themselves up. Staff accounts are created by an administrator; patient logins are issued at reception against a verified identity and revoked from the same screen.
Enforced in the database
Role scoping is not a UI decision. Rules live in the database, so a request that should not see a record does not see it — regardless of which client made it.
An audit trail that cannot be edited
Every write records who did it, to what, and when. Refunds, waivers, discharges and access changes additionally carry the reason typed at the time.
Your data, isolated
Each hospital runs against its own database instance with its own credentials. There is no shared table with a tenant column, so there is no query that can cross the boundary.
